Submitting a notice

Send a written notice to [email protected] identifying the copyrighted work, the allegedly infringing material and its location, your contact information, a good-faith statement, an accuracy-and-authority statement under penalty of perjury, and your physical or electronic signature.

CleanIQ response

CleanIQ may request missing information, preserve relevant records, notify the affected account, and remove or disable access to material CleanIQ controls when a complete notice appears valid. Because connected CRMs are customer-directed systems of record, CleanIQ may also direct a complainant or customer to the applicable CRM provider.

Counter-notice

An affected customer may submit a counter-notice identifying the removed material, consenting to the legally required jurisdiction, stating under penalty of perjury that removal resulted from mistake or misidentification, and providing a signature and contact information. Restoration, when applicable, follows statutory timing and any court notice.

Repeat infringement

CleanIQ may terminate accounts of repeat infringers in appropriate circumstances and may act on other intellectual-property violations under the Acceptable Use Policy.

Designated agent status

CleanIQ does not claim DMCA safe-harbor registration. A formal designated-agent notice will not be published unless Cain Family Insurance confirms the required entity, address, telephone, agent, alternate-name, and U.S. Copyright Office registration information.
